我国公立医院在经历了二十多年靠自身创收改善医生薪酬的激励机制之后，2017年人社部等四部门发布了改革试点指导意见，使薪酬制度改革成为公立医院改革的重点。本文从医疗服务的特殊性、医生行业组织形式和薪酬激励机制三个方面来探讨我国公立医院医生薪酬激励机制改革策略。医疗服务的特殊性使得自雇式成为国际医生行业组织形式的主流，但当医疗保障成为社会发展目标之后，医生和患者都必须面对有限公共资源的管理，而科学技术引起的医疗服务专业化使医生对医院的依赖越来越紧密，政府、医院和医生形成了从合同制到政府主办的各种医疗服务组织方式。但无论组织形式如何，由社会（政府）筹资方主导绩效考核，在有限资源下提高医疗服务价值是改革追求的共同目标。我国公立医院医生薪酬激励机制的主要问题是创收导向引起的诊疗方案费用高、效益低，但要转变为绩效导向则必须解决医院现行的经济运营体制，财政、价格和医保应该配套调整政策，让医院在追求服务绩效的改革中，收入同绩效同步增长。
,AbstractEN:The guide for public hospital salary reform jointly developed by four associated ministries focuses the public hospital reform on physician compensation. This article reviews the literature and summarizes physician compensation research in three aspects：special characteristics of medical care，physician organizational structure，and methods on payment incentive. The special characteristics of medical care makes self-employment as the mainstream of physicians’organization form. However，the relationship among government，hospital and physician is increasingly close when medical care gradually becomes public responsibility and both providers and patients are facing the challenge of limited resource. Regardless of the organizational form，a performance evaluation lead by the society and value improvement is the common goal of health system management. The main problem in physician payment incentive mechanism is high cost and low benefit driven by profit seeking incentive in public hospitals. To transfer from profit driven to performance evaluation，the government must reform the financing structure for public hospitals and enhance collaboration among the departments of health，finance，pricing and health insurance.

,AbstractEN:Economic level is an important measure of the gap between developed and underdeveloped countries. Behind the economic level，human capital and health have become an important factor in the development of a country. The health level of developed countries is obviously better than that of underdeveloped countries. The advantages of capital are obvious，and for less developed countries，how to promote economic growth by raising the people’s health level is an important issue for the developed countries. This article takes the relationship between health and economic growth as the starting point，integrates the research on the mechanism of health and poverty in foreign countries，analyzes the healthy endogenous technical progress and productive income，and puts forward that the study of health economics in China needs to pay attention to public health and public finances. The relationship between the individual health and the national economic development.

,AbstractEN:Objective：To understand the degree of awareness of the implementation of the major illness insurance system for urban and rural residents in the residents of Jiangyin and Taicang，and to provide evidence for the improvement of the serious illness insurance system and promote the allocation of public resources. Methods：The method of questionnaire survey was used to analyze the awareness and willingness of participation of the insured people in Jiangyin and Taicang，Jiangsu Province based on one-way analysis of variance and binary logistic regression. As a result，Taicang residents are more willing to continue to participate in health insurance than Jiangyin. Jiangyin residents’ willingness to participate in serious medical insurance is not strong；Jiangyin residents are relatively ignorant of the content of the policy；both residents hope to extract part of the basic medical insurance fund. The fund went out to pay for major illness insurance without additional burden of payment；Compared with Jiangyin residents，Taicang residents were more satisfied with the policy of serious illness insurance reimbursement for urban and rural residents. The significant factors affecting the insured people’s willingness to participate in major illness insurance include the type of household registration，whether to participate in the medical insurance for diseases，and the degree of understanding of the contents of the medical insurance policy for major diseases. Conclusion Strengthen policy publicity and improve the construction of serious illness insurance system for urban and rural residents. Change the traditional ideology.

,AbstractEN:Public hospitals need to solve the problems of “no separation of political and affairs”，“no separation of management and establishment” and management efficiency through the establishment of a corporate governance structure. How to establish and establish what type of corporate governance structure is the most important issue for public hospitals in practice. This article systematically analyzes the necessity of setting up a corporate governance structure in public hospitals from the perspective of commissioned agents. Through the analysis of domestic administrative commissioned，council，and board-type corporate governance structures，it is pointed out that in the current social conditions，the council should be the type of corporate governance is the mainstay. In some secondary public hospitals，it can be a trial the board of directors of corporate governance structure.

,AbstractEN:With the aging of the population and the growing scale of disabled elderly，the issue of long-term care has gradually become the focus of attention. At present，although many cities and regions have launched the pilot work of long-term care insurance system，the consensus of the system is relatively vague. It is urgent to put forward a national plan for social long-term care insurance system，in order to support the funds and services of the long-term care of most of the disabled elderly. Therefore，this paper discusses the basic framework of the social long-term care insurance system from the aspects of the system breakthrough，the coverage object and the service content，the financing，the demand assessment，and the management mechanism. And the system mentioned above will be suitable for the current national conditions and can be popularized all over the country.

,AbstractEN:Based on the culture cognition investigation of the public hospital among the medical staff，this article finds that there are many problems in cognition on the hospital culture connotation and function，and cognition on the current public hospital cultural construction of substance level，spirit level，behavior level and institutional level. Its reason mainly includes the superficial construction of mental hospital culture，the disjunction of material culture and spiritual culture，the deficiency of humanism core values，and the lack of constraint and timeliness on system construction. Countermeasure from four aspects should be put forward to foster the construction of public hospital culture：the humanistic spirit and harmonious hospital environment，mutual trust relationship in medical service and perfect management system.

,AbstractEN:“Attaching equal importance to Chinese medicine and Western medicine” is a basic policy of health work in China. In order to understand the actual situation of Chinese medicine participating in health services，this paper studies from two aspects：the Chinese medicine resources and Chinese medicine service utilization. Results show that Chinese medicine is an important part of the China health system，the status of Chinese medicine has been strengthened，integrated Chinese and Western Medicine hospital is increasing rapidly. To ensure and promote the development of Chinese medicine，it is necessary to make clear the orientation of Chinese medicine in the health system，establish a scientific talent training and incentive mechanism，strengthen the inheritance and innovation of Chinese medicine，and deepen the foreign cooperation and exchange.

,AbstractEN:Telemedicine has certain significance for alleviating the structural imbalance of medical resources in our country and the problem of unequal distribution of quality resources. With the development and coverage of China’s information and communication technologies，telemedicine is developing. However，there are problems such as the need to improve the standardization system in the development of telemedicine. On the basis of discussing the dynamic changes of telemedicine concepts，this article conducts an in-depth analysis of the development and application status of telemedicine in China from the five dimensions of telemedicine standardization system，laws and regulations，service pricing system，and the inclusion of medical insurance payment and marketization practices. Proposing relevant feasibility on the future development trend of telemedicine services.

,AbstractEN:Telemedicine can effectively alleviate resource tension. In recent years，telemedicine has been applied in various countries to alleviate the uneven distribution of medical resources. Telemedicine has developed rapidly. However，due to the lack of the implementation norms of telemedicine，the modes and effects of telemedicine in various places are different. Establishing a standardized telemedicine evaluation index system has become a key problem to be solved. With reference to the telemedicine evaluation model （The Model for Assessment of Telemedicine，MAST） established by the European Commission on，this paper study the evaluation of telemedicine applications from four aspects of clinical effectiveness，patient cognition，economic benefits，organizational change and points out the challenges and future prospects of telemedicine evaluation which lays a foundation for establishing the telemedicine evaluation in dexsystem.

,AbstractEN:The developed countries of Europe and America lead in the development of telemedicine. Factors such as support of policy，market-oriented operation and management，and integration with existing national health service system play an important role in promoting the development of telemedicine. At present，the lack of fiscal input，imperfect supervision system and ununified standards have restricted the development of telemedicine in China. This paper combs and compares the development path and status of telemedicine in the US，UK and Australia，in order to provide references for the development of telemedicine in China.
